About the MELD Score

The Model for End-Stage Liver Disease (MELD) is a reliable measure of mortality risk in patients with end-stage liver disease. It is used as a disease severity index to help prioritize allocation of organs for liver transplantation.

Formula

MELD = 3.78×ln(bilirubin) + 11.2×ln(INR) + 9.57×ln(creatinine) + 6.43

Note:

• Bilirubin and Creatinine are in mg/dL.
• If any value is less than 1, it is set to 1.0.
• If the patient has been on dialysis twice in the last week, or if Creatinine > 4.0, Creatinine is set to 4.0.
• The score is rounded to the nearest integer.
• The maximum MELD score is capped at 40.

Interpretation & Mortality

The MELD score correlates with 3-month mortality risk:

• Score ≤ 9: 1.9% mortality
• Score 10-19: 6.0% mortality
• Score 20-29: 19.6% mortality
• Score 30-39: 52.6% mortality
• Score ≥ 40: 71.3% mortality
